<br /> <br />BENEFIT <br />CON~rU[.TANT"'S- <br /> <br />Page 2 <br />February 10, 2009 <br />City of Sunny Isles Beach <br /> <br />Dental Insurance <br /> <br />The dental plan renewal option with Sun Life was at a 34% increase. This is due to the <br />groups higher than anticipated utilization and they are currently running at a loss. The <br />dental went out to market the same day as the medical to Principal, Cigna, Aetna, <br />Humana, Standard and Lincoln Financial. Lincoln Financial was able to propose the <br />current plan offered with Sun Life with the cunent rates. This had be discussed the <br />previous year that this would be approached as at that time Lincoln could not quote <br />against Guardian Life the previous carrier. <br /> <br />Vision Insurance <br /> <br />The vision plan renewal was under the CUlTent trend Eye Med Vision Care. It was <br />proposed to change from a two-tier rate to a four-tier rate. This resulted in the plan <br />lowering the single employee cost below cunen!. <br /> <br />Recommendation <br /> <br />It is our recommendation to change medical carriers to A vMed Health plans with the <br />triple option they have recommended. They would implement two HMO plans and a POS <br />plan for those out of the area or have children out of the area. The main HMO plan <br />would be at 2.5% below current.
