SITEWORK <br />21. DEMOBILIZATION <br /> <br />The scope of services shall include demolition of the existing pier structure and reconstruction of <br />the pier. The reconstruction shall also include constructing an outer shell for a 2,825 square foot <br />restaurant with seating for at least 105 people. The intent is to have the contractor provide a <br />viable holistic approach to the demolition and construction of the Pier. The estimated <br />construction budget is $3 million dollars. <br /> <br />1.6 SUBMITTAL REQUIREMENTS <br /> <br />All submittals by contractors shall contain no more than fifteen (15) pages specifically <br />addressing the following issues. Item numbers 9 and 10 below will not be considered a part of <br />the fifteen (15) pages. Any contractor wishing to provide the services described in Section 1.5 <br />above must submit one (1) original and four (4) additional complete copies that include, at a <br />minimum, the following documentation: <br /> <br />1. Name, address and company, including, but not limited to, a business overview, financial <br />state of the business, annual revenue for past two years, and names and addresses of <br />persons having financial interest in the firm. <br /> <br />2. Provide a narrative description of current and previous experience demolishing and/or <br />constructing a project similar to this RFP. <br /> <br />3. A list of all projects (including government projects) completed or in process for the past <br />five (5) years, and a corresponding list of client references with contact names and <br />telephone numbers. <br /> <br />4. Proof of authorization to transact business in Florida from the Florida Secretary of State, <br />from the prime as well as supporting firms. <br /> <br />5. The name, title, qualifications, experience, and all contact information of the proposed <br />contact person and proposed personnel to be assigned to this project. <br /> <br />5 <br />
