<br />6. Provide a detailed description of the Proposer's procedures for coordinating the <br />demolition and construction of the Pier, including the project implementation schedule, <br />identifying events and the estimated number of calendar days to achieve completion. <br /> <br />7. Provide a detailed financial cost breakdown of the total cost involved in demolishing and <br />constructing the Pier. <br /> <br />8. Provide documentation demonstrating Proposer's financial strength and financial capability to <br />provide the demolition and construction services in this RFP. Such documentation should <br />include Proposer's most recent certified financial statements as of a date not earlier than the end <br />of the Proposer's preceding official tax accounting period, with a statement in writing, signed by <br />a duly authorized representative, stating that the present financial condition is materially the <br />same as that shown on the balance sheet and income statement submitted, or with an explanation <br />for a material change in the financial condition. If certified financial statements are not <br />available, provide latest available financial statements (balance sheet and income statement) and <br />letters of credit availability from accredited financial institutions, or other relevant <br />documentation. <br /> <br />9. Sworn statement pursuant to Section 287.133(3)(A), Florida Statutes, on Public Entity <br />Crime, a copy of which is attached hereto. <br /> <br />10. Non-collusive Affidavit, a copy of which is attached hereto. <br /> <br />1. 7 LIABILITY INSURANCE <br /> <br />Contractor shall, at its sole cost and expense, during the period of any work being performed <br />under this Agreement, procure and maintain the following minimum insurance coverage to <br />protect the City and Contractor against all loss, claims, damage and liabilities caused by <br />Contractor, its agents, sub- Contractors or employees, as indicated below: <br /> <br />o Comprehensive General Liability ("CGL") insurance, with minimum limits of <br />One Million Dollars ($1,000,000) per occurrence, combined single limit for <br />Bodily Injury Liability and Property Damage Liability, and Two Million Dollars <br />($2,000,000) aggregate. Coverage must be afforded on a form no more restrictive <br />than the latest edition of the Comprehensive General Liability policy, without <br />restrictive endorsements, as filed by the Insurance Services Office, and must <br />incl ude: <br /> <br />· CGL Required Endorsements include the following: <br /> <br />I. Employees included as insured <br />II. Independent Contractor's Coverage <br />III. Contractual Liability <br />IV. Waiver of Subrogation <br />v. Premises and/or Operations <br />VI. Explosion, Collapse and Underground Hazards <br />VII. Loading and Unloading <br /> <br />6 <br />
