A network of independent agents backs <br />up sales and service to small groups. <br />Humana offers its members award - winning programs <br />in preventive care, early intervention <br />and chronic disease intervention. <br />Providing solutions to our <br />members' health problems <br />Humana takes its members' health very seriously. <br />The company puts extensive energy and resources into <br />providing access to services that seek to help people get <br />well and prevent them from getting sick in the first place. <br />Members enrolled in Humana's end stage renal disease <br />(ESRD) intervention program administered by RMS <br />Disease Management Inc. exceeded national averages <br />for expected outcomes.The RMS program results are <br />measured against benchmarks established by the <br />Health Care Financing Administration. <br />Ninety-three percent of Humana members in the <br />RMS program achieved or exceeded hemodialysis <br />adequacy targets versus the national average of <br />78 percent. <br />More than 1,500 members enrolled in Humana's <br />congestive heart failure program (CHF) with an <br />average enrollment of 12 and one half months. <br />Eighty-eight percent of Humana members enrolled in <br />the CHF program are on appropriate vasodilator <br />medication, with 60 percent of members receiving <br />Continued on back side <br />
