It is <br />also our desire to offer viable alternatives to develop the most comprehensive and <br />competitive program. In regard to the group health, you will note that various types of <br />plans and premiums are offered by two carriers, Aetna USHealthcare, Inc., and Humana, <br />Inc. Aetna USHealthcare, Inc., offers combinations of HMO, POS and US Access, an <br />"Open Access" plan that does not require a referral from the primary care physician. <br />Humana Inc. can combine the HMO with the PPO plan. The Humana, Inc., POS plan is <br />not combinable with other plans. <br />We are presenting an independent comprehensive Vision Plan with EyeMed, a division <br />of Luxotica, Inc. The Dental Plan is proposed with Guardian Life Insurance Company of <br />America. <br />In order to determine the best plan, or combination of plans, it is important to carefully <br />review the various benefit and premium summaries provided in the proposal.
