4.2 All proposals shall be submitted on the Bid Form and shall include all requested <br />documentation, including photos and detailed specifications of all equipment, Warranty <br />documents and a completed Public Entity Crime Form. <br />4.3 The proposals shall contain two separate sections. The first section should address and be <br />limited to the consultant's qualifications. The second section should address the specific <br />work proposed by the consultant. <br />4.4 Mail or deliver six (6) original proposals (faxed proposals will be rejected) to: <br />Christopher J. Russo <br />City of Sunny Isles Beach <br />17070 Collins Avenue, Suite 250 <br />Sunny Isles Beach, FL 33160 <br />4.5 The City reserves the right to accept or reject any or all proposals received. <br />4.6 Proposals shall be publicly opened at City Hall, 17070 Collins Avenue, Suite 250 on <br />Friday, December 20, 2002 at 10:00 a.m. <br />4.7 Any additional information can be obtained by calling the City Manager at (305) 947 -0606. <br />4.8 The City of Sunny Isles Beach is tax exempt (Certificate #23 -00- 477131 -54C). <br />4.9 The bidders are required and shall submit proposals for the provision of municipal services <br />which, at a minimum, shall include the following: <br />A) A description, not to exceed three 8 '/2" x 11" single spaced typed pages, of the <br />bidder's work program. <br />B) Describe the organization that you plan to create for the purpose of working on the <br />project, including staff and their duties, location of offices and methods of <br />communicating information to the City Manager. Describe how consultants will be <br />integrated into the organization of the project. <br />C) Provide an organization chart showing functional relationship between staff. Show <br />the lines of authorization and assignments of responsibility. Indicate the expected <br />participation of top management. <br />4.10 Cancellation of Request for Qualifications. A Request for Qualifications or other <br />solicitation may be cancelled, or any or all bidder(s) may be rejected in whole or in part, as <br />may be specified in the solicitation, with City's sole discretion. The reasons therefore shall <br />be made part of the contract file. Each solicitation issued by the City shall state that the <br />solicitation may be cancelled and that any bidder may be rejected, in whole or in part, <br />within the sole discretion of the City. Notice of cancellation shall be sent to all businesses <br />solicited. The City shall give any bidder the opportunity to compete on any re- solicitation <br />or any future procurement of similar items. <br />4.11 Correction or Withdrawal of Proposals; Cancellation of Awards. In general, proposals shall <br />be unconditionally accepted without alteration or correction, except as authorized in this <br />Ordinance. However, correction or withdrawal of inadvertently erroneous proposals before <br />or after bid opening, or cancellation of awards or contracts based on such proposal <br />Page 4 of 10 <br />Municipal Contract Services Rfq <br />
