<br />Traffic Control <br />As discussed in other sections, Ceres requires and will provide certified traffic control personnel for debris <br />collection, transportation, and processing operations. Competent and qualified personnel will be trained in <br />traffic control procedures and will be provided necessary safety equipment and communication devices. <br />Traffic control personnel will generally be placed at either end of a work zone in order to properly control <br />the flow of traffic into and out of the work zone. <br /> <br />Site Restoration <br /> <br />The Site Restoration and Environmental Survey Plan will ensure that restoration of the site will meet the <br />owner's requirements and local regulations. In addition to site cleanup and removal of all debris, the <br />Restoration Plan will include requirements for achieving ground cover through topsoil and seeding <br />specifications. Other requirements may be mandated by the Erosion Control Plan, such as maintenance <br />of straw bales, retention ponds, or erosion control fencing until ground cover is established. An outside <br />independent party may be employed to conduct a post utilization environmental survey in order to ensure <br />satisfactory site conditions. Site closure is normally accomplished within 30 days of receipt of the last load <br />of disaster related debris. <br /> <br />Demobilization <br /> <br />The OM shall prepare a demobilization checklist that includes a punch list of items to be completed by his <br />staff. The Punch List may include items such as arrangement for future maintenance of erosion control <br />measures. <br /> <br />DOCUMENTA nON - FIELD OPERA TIONS <br /> <br />Production Reporting <br /> <br />Ceres has developed specific internal <br />procedures to ensure proper audit-quality <br />documentation of daily project activities is <br />captured and provided to the City. These <br />include: project tracking forms, load <br />tickets, truck certification logs, production <br />logs, shift inspection checklists, safety <br />meeting report forms, daily crew reports, <br />and various equipment usage reports. <br />Other reports are prepared and submitted <br />to document project activities, progress, <br />and quality control. These reports may <br />include, but may not be limited to: <br /> <br />~-~--- - - _..._~----, <br /> <br />. The zone, Section number, and street where debris <br />removal operations were conducted and/or completed. <br />. The total number of personnel engaged in debris <br />management and position or activity <br />. Daily and aggregated man-hours <br />. Then number of loaders and debris hauling vehicles in <br />operation <br />. Hours of use of trucks and equipment <br />. The daily and aggregate volumes of debris, by type, <br />removed and processed <br />. The number, name and location of each debris <br />management site in operation to include numbers and <br />types of reduction equipment in use I <br />. Mulching machines in operation , <br />. The percent completion of the project <br />. The estimated completion date <br />. Any inspections conducted by federal, state or local <br />government agencies <br />. Any testing performed and/or test results <br />. Quality control phases implemented, as applicable <br />. Any corrective actions implemented <br />. Any damage to private property caused by contractor <br />operations <br />. Any reports of damage or claims made by citizens <br />. Other information as may be required to fully and <br />completely describe the contractor's daily operations <br />. A weekly summary of the information from the daily <br />reports <br />. A final project summary report to describe all debris <br />management activities conducted and conformance to <br />contract specifications <br />. Additional information or reports as necessary to <br />adequately document the conduct of debris management <br />operations. CERES_027 <br /> <br />Quality Control <br /> <br />Daily Contractor Production and Quality <br />Control reports will be completed each <br />day of work and available the following <br />work morning to the City. Original reports <br />are maintained in the Mobile Command <br />Center and daily reconciliation reports are <br />generated to verify information reported <br />on load tickets to information reported on <br />daily production reports.
