<br />our Governmental customers and has been used in the past for audits by such Federal agencies as the " <br />OCAA. ~ <br /> <br />Any private property ROE's that are issued are also integrated into our database system. It is our policy to <br />take before and after pictures for all ROE properties. These pictures are linked in the database with the <br />ROE records, which are both accessible over the internet. <br /> <br />Prior to initial use of our system, authorized Ceres personnel, and contract representatives as required, <br />will inspect hauling trucks. Only pre-approved trucks will be received at the TDMS. Approval shall include <br />documentation of truck identification and insurance, safety requirements, and measured cubic yardage <br />capacity. A unique approval number shall be assigned to the truck and posted on the truck along with <br />measured capacity. All units hauling debris are required to be "measured in" prior to commencement of <br />work. The hauling uniUtruck/ trailer certification procedure is mandatory and will be administered by <br />quality control representatives of Ceres and the Client or the Client's designated authority(s). A Truck <br />Certification Log Sheet will be created for each hauling uniUtruck/trailer. Unit specific information along <br />with Year, Make, Address, Photograph, Model, License Plate information, Driver Name, and signatures <br />will be recorded on the log. At this time a unique identifier will be assigned to the unit. The identifier will <br />consist of both letters and numbers. Each Subcontractor will be assigned a Letter Code unique to the <br />Subcontractor. The number portion of the identifier will be sequential (The first truck measured is 100; the <br />second truck measured is 101; the third truck measured is 102; and so forth). A given number will not be <br />assigned to more than one hauling unit (there is only one 101, 102, 103, etc....). Combining the <br />subcontractor identifier with the number would result in the unique identification for each unit. ACE100, for <br />"Ace Trucking Company;" TTS101 for "Toms Tree Service." Using both a lettering and numbering system <br />for the Truck Identification will minimize any potential confusion by having both the volume (CY) and truck <br />number on the same label. The truck identification always has letters in front of the numbers. Truck <br />Certification Logs will be maintained by Quality Control staff. The log shall be maintained and available to <br />TDMS inspection personnel regarding truck approvals, approval number, capacity, and other pertinent <br />information. <br /> <br />Upon certification, the identification and volume information is written (with permanent marker) on the <br />label (placard). Ceres uses labels pre-printed with our name and blanks for identification number and <br />volume. The label is then immediately affixed to a standard, visible location on both sides of the <br />truck/trailer/hauling unit from which it cannot be removed without destroying the label. All equipment is <br />subject to inspection by the Client. <br /> <br />Dispatch records will be maintained for the duration of the project. Records will include date and time of <br />dispatch, crew and unit identifier, and status of assigned section (In Progress, Completed, etc.). Typically, <br />one contractor will be assigned to a given section. Sections may be comprised of individual developments <br />or combinations thereof. Accurate and thorough Dispatch Logs enable the identification of any potential <br />issues and the responsible party. <br /> <br />Additionally, all supervisors will conduct weekly toolbox meetings and develop activity hazard analyses in <br />compliance with the corporate Health and Safety Plan, and these meetings are documented. <br /> <br />DOCUMENTATION - FIELD OPERA TIONS <br /> <br />Production Reporting <br />Ceres has developed specific procedures to ensure proper and thorough documentation of daily project <br />activities and adherence to strict quality control requirements. Daily documentation required for each <br />debris management project will meet or exceed contractual requirements and FEMA or other agency <br />requirements. Ceres has developed project-tracking forms to ensure accurate reporting. Some of these <br />forms include: load tickets, truck certification logs, production logs, shift inspection checklists, safety <br />meeting report forms, daily crew reports, and various equipment usage reports. Other reports are <br />prepared and submitted to document project activities, progress, and quality control. These reports may <br />include, but may not be limited to: <br /> <br />Quality Control <br />Daily Contractor Production and Quality Control reports are completed and available the following work <br />morning to the client or other designated authority.
