<br />- <br /> <br />o <br /> <br />2. Computerize a detailed listing of all affected properties, to include: <br />A. Description <br />B. Damages <br />C. Owner with contact information <br />D. Identification of Task Order <br />E. Progress of Work <br />F. Sign-off and release <br />G. Any peculiarities pertaining to entry or safety <br />H. Certification that property is vacant and all valuables have been removed <br />if demolition to occur. <br />I. Location of unseen obstructions <br />3. Obtain signed right of entry agreement from owner to include: <br />A. Right of Entry Agreement <br />B. Hold Harmless Agreement (to protect contractor, subcontractors, <br />County/City, and FEMA from claims) <br />C. Non-duplication of Benefits fonn to ensure owner does not receive both <br />federal assistance and insurance proceeds for the same work. <br />4. Assess the property thoroughly with Government Agency and obtain detailed <br />scope of demolition and/or work to be performed from the Government for <br />reach site. <br />5. Obtain proper pennits and/or Condemnation Certificates for work, if required. <br />6. Prepare drawings and/or photographs of each site from multiple angles to <br />document condition of property prior to entry. <br />7. Enter photographs into computer under particular ROE file. <br />8. Prepare and maintain computerized schedule and daily progress of work. <br />Update daily for a continuous mechanism to track programs and completion <br />of all ROE's. <br />9. Government shall clearly designate and physically make each property to be <br />entered and/or demolished. <br />10. Clearly locate, mark and/or protect all utilities. Tenninate utilities at street if <br />part of task order. Electricity and gas to be terminated by utility company, if <br />appropriate. <br />II. Dispatch Superintendent and Subcontract crew to site to perfonn debris <br />removal, demolition and/or such other tasks on the private property as may be <br />directed by the Government Agency. <br /> <br />Q <br /> <br />Q <br /> <br />C. FIELD SUPERVISORS/CREW FOREMEN/SUBCONTRACTORS <br />I. Field supervisors and subcontractors will report to the senior field supervisor <br />or project manager. <br />2. All supervisors will be responsible to ensure work is conducted only on those <br />properties designated by the government. Supervisors will not allow work to <br />commence in additional areas until directed by government task order. <br />3. Supervisors shall be responsible for the safety of all personnel and equipment. <br />4. Supervisors will be responsible for collection of daily personnel and <br />equipment time logs, and their distribution to the Company designated <br /> <br />65 <br />
