<br />o <br /> <br />C) <br /> <br />c <br /> <br />prior to the commencement of work (or any single phase of work); (2) the continuing <br />instruction/monitoring of each contractor, subcontractor, supplier and employee in the safe <br />operation of their specific area of responsibility using the proper tools and in accordance with the <br />safety procedures and guidelines outlines in United States Army Corps of Engineers Manual EM <br />385-1-1 revised October 1998 to insure that all work is performed in a safe manner. Through <br />careful planning, hazard recognition and control, safety indoctrination and training, and rigorous <br />attention to safety procedures, we shall ensure the health and safety or personnel at our work <br />sites and the public adjacent to our work sites. <br /> <br />No person shall be required or instructed to work in surroundings or under conditions that are <br />unsafe or dangerous to his/her health. Any person aware of an unsafe or dangerous condition <br />shall report the condition to his/her supervisor immediately. The particular operation will be <br />stopped; the project manager will appoint a competent individual to investigate the condition and <br />make corrections prior to restart of the operation. All information shall be recorded, maintained <br />in the project file. A copy of the manual is available upon request. <br /> <br />Safety and health meetings shall be conducted once a month for all supervisors on the project <br />location and once a week by supervisors (foreman) for all workers. The meetings shall be <br />documented by the Safety Officer. <br /> <br />The minimum information included in the report shall be (1) the date of the meetings; (2) name, <br />social security number, and signature of attending individual(s); (3) the name of the <br />individual(s) conducting the meeting. Copies of the safety manifest will be kept on file for a <br />period of one year and shall be furnished to the designated authority upon written request. <br /> <br />The safety and health indoctrination and training meetings shall be based upon the contractor <br />Safety Program and the United States Army Corps of Engineers Safety and Health Requirements <br />Manual EM 385-1-1, October 1998. Safety and health subjects, at a minimum, shall include: <br />. Requirements and responsibilities for accident prevention and maintaining safe and <br />healthful work environments <br />. General safety and health policy and procedures and pertinent provisions of EM 385-1-1 <br />. Employee and supervisor responsibilities for reporting all accidents <br />. Provisions for medical facilities and emergency response and procedures for obtaining <br />medical treatment or emergency assistance <br />. Procedures for reporting a correcting unsafe conditions or practices <br />. Job hazards and the means to control/eliminate those hazards, including applicable job <br />and/or activity hazard analyses <br />. Job hazards communication <br /> <br />HEALTH AND SAFETY PROVISION <br />Accident Reporting <br />All supervisory personnel, foreman and employees will be instructed to report accidents to the <br />Project Manager. Employees are responsible for reporting all injuries or occupational-related <br />illnesses as soon as possible to his/her immediate supervisor. The contractor's office will record <br />all incidents, along with the treatment provided. Reports will contain the following information: <br />. Name <br /> <br />47 <br />
