Item by Item Disclosure <br />H.A.C.C. will provide a full turn- key operation of the following Project <br />Requirements; <br />1. Concrete removal and paver installation work is excluded at all ADA ramps lo- <br />cated at the pedestrian crossings. The cost of providing all necessary labor, speci- <br />fied materials and equipment to paint the ADA ramps with approved colors and <br />approved pattern, complete as required by the Owner (City if sunny Isles) included <br />in our bid. <br />2. H.A.C.C. will provide owner approved FDOT/ Miami Dade County, ADA de- <br />tectable warning surfaces at all crosswalks. All materials used on this project will <br />be approved by the owner via material shop drawings prior to purchase. <br />3. H.A.C.C. will submit owner furnished drawings for new permits from FDOT <br />and subsequently to other AHJ. <br />4. H.A.C.C. is familiar with the FDOT variation attached to RFP. The City engi- <br />neer will provide all necessary information to H.A.C.C.. Contractor will apply and <br />obtain a new permit variation if required by FDOT. <br />5. H.A.C.C. will place owner approved pavers in the owner approved exact pattern <br />and color as in other locations through -out the City along Collins Ave. H.A.C.C. <br />will field verify all dimensions and copy the same for the pavers proposed under <br />this RFP. <br />6. H.A.C.C. shall replace all drive ways indicated in RFP with owne approve pav- <br />ers at elevations and pitches consistent with FDOT Standard indexes. <br />7. H.A.C.C. shall obtain any new permits and maintain existing active permits <br />required by RFP for the duration of the project. Contractor will comply with all <br />permit requirements and ensure the cost associated with that compliance with the <br />RFP is included in the contractor's proposal. <br />8. H.A.C.C. is responsible for obtaining all necessary permits required by this RFP, <br />which include but are not limited to FDOT, Miami Dade County and the City of <br />Sunny Isles. H.A.C.C. will ensure that the permits remain active for the project <br />duration. Cost is included our bid. <br />9.
