purpose when entering departments other than those they are assigned during <br /> normal business hours. <br /> 4) Each department director will be responsible for working with the Human <br /> Resources Department to establish and set forth the appropriate security access <br /> for each employee based on their respective job functions. The possession and <br /> proper use of the ID card is the responsibility of the employee and shall be <br /> safeguarded at all times. <br /> 5) In the event of a lost ID card, Human Resources will deactivate and issue a new <br /> ID card. An administrative fee of $10.00 will be charged for all lost cards. <br /> 6) In the event of a damaged ID card, Human Resources will deactivate and issue a <br /> new employee ID card. Damaged ID cards must be turned in to Human Resources. <br /> There will be no fees for a replacement ID card. <br /> 7) When an individual terminates employment, or otherwise ends their assignment <br /> with the City, the ID card shall be returned it to the Human Resources Department. <br /> In cases where an individual is put on administrative leave or suspension, the <br /> Human Resources Department shall disable the individual's ID <br /> Card. <br /> 8) ID Card Holder Responsibilities: <br /> a. Do not lend your ID to anyone. <br /> b. Do not allow unauthorized individuals into any secure area (i.e. visitors, <br /> former employees, etc.) <br /> c. Do not hold or prop secured doors open or leave doors open after you leave. <br /> d. Do not leave ID on dash of vehicle or other locations where exposed to <br /> extreme temperatures. <br /> e. Do not fold, bend pry open or mutilate your ID. <br /> f. Do not use your ID improperly. <br /> g. Do not leave your ID unattended. <br /> h. Immediately notify your supervisor if your ID is no longer in your possession. <br /> i. Immediately notify your supervisor of any difficulties or problems with any <br /> ID. <br /> 9) Because the policy and procedures described herein are intended to provide for <br /> the safety and security of City employees, all employees are expected to fully <br /> comply with all provisions of this policy. The employee shall be given an initial <br /> warning if the employee has failed to comply with this policy.
