City of Sunny Isles Beach <br /> RFP 18-04-02 Disaster Debris Management and Disposal Services <br /> 0 ° Review of records system for applicability to Federal and State Requirements <br /> ■ Orientation and training of client personnel on documentation requirements <br /> • Assist in the establishment of the "Clerk of Records" <br /> • Claim Documentation <br /> • Public Service Announcements <br /> Production Reporting <br /> Ceres has developed specific procedures to ensure proper and thorough documentation of daily project <br /> activities and adherence to strict quality control requirements. Daily documentation required for each debris <br /> management project will meet or exceed contractual, FEMA or other agency requirements. Ceres has <br /> developed project-tracking forms to ensure accurate reporting. In addition to the forms already mentioned <br /> these forms include: truck certification logs, production logs, shift inspection checklists, safety meeting <br /> report forms, daily crew reports, and various equipment usage reports. <br /> Quality Control <br /> Daily Contractor Production and Quality Control reports are completed and available the following work <br /> morning to the client or other designated authority. Original reports are maintained in the Mobile Command <br /> Center and daily reconciliation reports are generated to verify information reported on load tickets to <br /> information reported on daily production reports. The Project Manager and Project QC Manager monitor <br /> information contained in the Daily Quality Control reports to ensure project activities conform to contractual <br /> requirements and that an acceptable level of project quality and workmanship is provided to the client. All <br /> records, certifications, and reports are converted into digital documents that are stored securely off-site on <br /> Ceres computer servers and are available to management and other project personnel on a need to know <br /> basis. <br /> Formalized quality control procedures are applied to each project to ensure documentation procedures are <br /> properly and fully implemented and to ensure conformance to project specifications. All Ceres employees, <br /> subcontractors, and suppliers are subject to the provisions of the QC Program. For each project, a Quality <br /> ki I Control Plan is specifically developed to detail the QC organization, individual responsibilities, monitoring <br /> procedures of activities and subcontractor activities, documentation requirements for Ceres personnel and <br /> all subcontractors, control phases or procedures, and identification and correction procedures for non- <br /> conforming activities. The remedies for non-conformance include termination. Exceptional quality control of <br /> each project promotes efficiency and avoids investigation and other potential losses. <br /> Dispatch Records <br /> Dispatch records will be maintained for the duration of the project. Records include date and time of <br /> dispatch, crew and unit identifier, and status of assigned section (In Progress, Completed, etc.). Typically, <br /> one contractor will be assigned to a given section. Sections may be comprised of individual developments <br /> or combinations thereof. Accurate and thorough Dispatch Logs enable the identification of any potential <br /> issues and the responsible party. <br /> Additionally, all supervisors will conduct weekly toolbox meetings and develop activity hazard analyses in <br /> compliance with the corporate Health and Safety Plan, and these meetings are documented. <br /> 4i) <br /> C �� Tab 7 Personnel Experience and Qualification <br /> Section 3 FEMA Knowledge and Experience <br /> ENVIRONMENTAL Page 7.3-7 <br />
