REQUEST FOR QUALIFICATIONS NO. 20-07-01 <br />CONTINUING PROFESSIONAL CONSULTING SERVICES (CCNA) <br /> <br /> <br /> <br /> <br />TAB F: FIRM REFERENCES AND SIMILAR WORK COMPLETED <br /> <br />Installation of New Redundant Bypass Line (Zone 4B & 4C) - 54” Force Main <br />Fort Lauderdale, Florida <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br />Description of Work: <br />The project involved the installation of 54” nominal OD <br />HDPE Force Main by Horizontal Directional Drill (HDD), with <br />sections of open cut trench installation of 16” HDPE Force <br />Main. The total length of Horizontal Directional Drill (HDD) <br />54” OD HDPE Force Main is 3,223 Linear Feet (LF) in length <br />which was proposed to minimize the disturbance to the <br />community and limit the amount of pavement restoration, <br />with an additional 653 Linear Feet (LF) of 16” HDPE Force <br />Main installed by open cut trench. CTA provided full time <br />construction management and inspection staff to monitor <br />contractor adherence to schedule, resolve disputes and <br />conflicts, process and respond to requests for information <br />(RFIs), evaluate Change Order Requests and process valid <br />requests, ensure that construction was performed in <br />conformance with plans and specifications, review as-built <br />plans and shop drawings, process monthly pay applications, <br />complete substantial and final inspections, and certify the <br />project. <br />The City of Fort Lauderdale awarded the Design Build <br />Contract for the installation of redundant piping to DMSI, <br />Inc.
