REQUEST FOR QUALIFICATIONS NO. 20-07-01 <br />CONTINUING PROFESSIONAL CONSULTING SERVICES (CCNA) <br /> <br /> <br /> <br /> <br />TAB F: FIRM REFERENCES AND SIMILAR WORK COMPLETED <br /> <br />Don Shaver (continued) <br />SW 36th Street | Davie, Florida | Construction Manager <br />Craven Thompson provided civil engineering, CEI, and miscellaneous survey services for the project. SW <br />36th Street was designed as a two (2) lane divided boulevard from College Avenue to SW 76th Avenue <br />within a 75-foot public right-of-way width. In connection with this improvement, any portions of SW 36th <br />Street dedicated to the Town by NSU for roadway purposes. <br />Resurfacing and ADA Sidewalk Improvements | Miami Gardens, Florida | Construction Manager <br />The resurfacing and ADA walk improvements included approximately 24,000 linear feet of road <br />throughout the city in three neighborhoods. The neighborhoods consist of a mixture of single and multi- <br />family residential dwellings. Significant amounts of the existing roadway infrastructure within these <br />neighborhoods did not meet current ADA requirements, which warranted attention in order to conform to <br />current standards. The project involved coordination with FDOT for LAP funding and design criteria, and <br />the development of construction drawings and specifications. <br />NE 14th Avenue Streetscape Improvements | Hallandale Beach, Florida | Construction Manager for the <br />Mill and overlay of the entire roadway (including pavement markings); on east side of NE 14th Avenue, <br />mill and overlay off-street parking (including pavement markings) and provide new Type ‘D’ curbed <br />landscaped islands. <br />Cypress Park | Sunrise, Florida | Construction Manager <br />The site plan was designed to preserve and enhance all of the existing natural resources while providing <br />a beautiful, safe and functional passive park for the residents. The park contains a walking/jogging trail <br />with exercise stations, an elaborately themed Splash Pad / “Sprayground”, pervious parking area, a nine- <br />hole disc golf course, multiple custom picnic pavilions and a custom restroom facility as well as <br />environmental educational materials. As you enter the park a rec ycled glass river guides the visitors to <br />the “Sunrise Swamp”- Splash Pad and there amongst the lush native vegetation the alligators are there to <br />spray you with water. Craven Thompson was the prime consultant responsible for the park design as well <br />as the survey, preparation of the open-space park management plan, the design workshops, conceptual <br />site design, construction documents and construction management. <br />Miramar Historic Area Complete Streets Phase II | Miramar, Florida | Construction Manager <br />Mr. Shaver was responsible for Construction Management for Phase II of their Historical Area Complete <br />Streets Project which included six (6) streets with the following improvements: sidewalks, ADA ramps, <br />crosswalks, pedestrian level lighting, landscaping and hardscape. <br />Bluesten Park | Hallandale Beach, Florida | Construction Manager <br />Design and construction of a 42,000 sf Recreation Center with Pool and Splash Play Area, Soccer/Multi- <br />use Field, three (3) baseball Fields, three (3) basketball courts, two (2) tennis courts, two (2) racquetball <br />courts, boundless ADA inclusive playground, walking trails and pavilions; and full promenade streetscape <br />design for streets for parallel parking. <br />SW 41st Place Roadway and Infrastructure Improvements | Davie, Florida | Construction Manager <br />This project included the demolition of existing utilities, installation of large diameter storm drain, water <br />main, sanitary sewer and roadway/sidewalk as part of Phase I of the Eastside Master Drainage Study and <br />Eastside water and sanitary sewer improvements. Craven Thompson provided design and permitting <br />services for roadway, water, sanitary and drainage as well as CEI services including shop drawing review, <br />meeting coordination, field observation, review and coordination of pay requests, change orders and <br />Requests for Information. Services also included Project close-out, Substantial and Final Completion, <br />review of Record Drawings and Certification of Permits to applicable agencies. <br /> <br />F | Page 47
