TRIDENT <br />ENVIRONMENTAL <br />marine design and permitting <br />April 5, 2022 <br />Contact Information: <br />Keith Associates <br />301 E Atlantic Blvd <br />Pompano Beach, FL 33060 <br />Attn: Mark Castano, PE <br />954-788-3400 <br />Exhibit 'B' Bibi Villazon <br />Project Information: <br />Bella Vista Bay Park <br />500 Sunny Isles Blvd <br />Sunny Isles Beach, FI <br />Job Description: <br />New Concrete Seawall <br />PROPOSAL /CONTRACT <br />President <br />305 244 0595 <br />bibi@tridentenv.com <br />www.tridentenv.com <br />This proposal is based on your request for design and permitting services for the following scope <br />of work: <br />Approx. 180 LF of New Concrete Seawall, including panels, batter piles and new concrete cap. <br />New cap elevation to match adjacent property at Chabad. <br />NOTE FINAL DESIGN IS CONTINGENT ON AGENCY APPROVALS AND MAY CHANGE DUE TO <br />MARINE RESOURCES, WATER DEPTHS AND OTHER FACTORS. <br />This proposal includes signed sealed engineered construction drawings, specifications and <br />processing of these drawings thru the required agencies, including: DERM, FDEP, ACOE and your <br />local Municipality. The following is our scope of work: <br />DESIGN AND PERMIT PHASE <br />a. Provide detailed drawings and specifications for County, State, Federal and Municipality <br />Approval, including plans, cross sections, layout, details and calculations. <br />b. Process all applications to agencies as necessary to obtain building permit as well as <br />environmental permits. <br />c. Surveys, Tree Surveys, As -built surveys, Bathymetric Surveys, and Geotechnical Reports <br />including pile length recommendations, as may be required to provide additional <br />information to the permit agencies, shall be provided to us and are not included in this <br />contract. <br />TOTAL DESIGN AND PERMIT PHASE $ 6,500.00 <br />d. Agency fees required by the permitting agencies are NOT included in this proposal and <br />are payable by Owner directly to the permit agencies.
