John Moriarty and Associates of Florida, Inc. <br />Jason Roth - Senior PM - Authorized Representative <br />1942 Tyler Street, Hollywood, FL 33021 <br />PH 561.213.6524 <br />JRoth@jmaf.net <br />Project Location:19051 Collins Avenue, Sunny Isles Beach, FL 33160 <br />Estimated Start Date: April 2027 <br />Estimated Completion Date: April 2030 <br />Hours of Operation: Construction hours mandated by City of Sunny Isles Beach, plus certain <br />extended hours, as requested, and approved by the City. <br />Project Task: Describe project, include equipment to be used. <br />Note: Copy of liability insurance with the City of Sunny Isles Beach as an additional <br />insured. <br />Traffic Control: Will this operation require occupying any traffic lanes, parking lanes, <br />beachwalks or any other public right -of way? If yes, closures shall be per the regulations <br />described in the Maintenance of Traffic (MOT) from the Florida Department of <br />Transportation (FDOT). Please note that a Lane Closure Permit from the Code <br />Compliance Department must be obtained prior to public right-of-way use. <br />Parking: Indicate the number of workers and the areas where their vehicles will be parked <br />through the duration of this phase of the project. Any parking displacement must be <br />addressed accordingly and in advance. <br />Clean-up: Describe the measures that will be taken to ensure that the work site and <br />public right-of-way will be maintained (including dust control, swale on driveways to <br />prevent water leaks to the street, gutter buddies, rocks at driveways, etc.) <br />Fencing: Construction fencing must be constructed of durable material, and additional <br />silt fencing for additional protection. Gates should be steady and properly secure during <br />non -working hours. <br />Provide erosion sediment control plans, if applicable. <br />Material Storage Location: Describe the location for material storage (if needed). Any <br />removal or relocation of sand, must be approved separately. <br />320 <br />
