A listing of all subcontractors, if any, and the portion of the contract they will <br />perform must be submitted with the proposal. If sub -contractors are stated, this does not <br />relieve the Vendor from the prime responsibility for full and complete satisfactory and <br />acceptable performance under any awarded contract. <br />1.3.5 PAYMENT <br />Payment and statement for the previous month's gross receipts shall be received by the <br />City by the fifteenth (15th) of the following month. If the Vendor fails to make this <br />payment within five (5) days of the due date, there shall be an additional charge of $50.00 <br />payable to the City for such late payment. The late fee shall be applied one time per <br />month. <br />The Vendor shall be responsible for collecting and remitting all sales tax to the State of <br />Florida. Vendors are advised that payments made to the City of Pembroke Pines for the <br />right to operate any vending are considered taxable as rental payments. Therefore, the <br />rental payments due to the City on the fifteenth (15th) of the month will include <br />applicable sales tax. The Vendor is responsible for any and all real property taxes, special <br />assessments, and any other taxes imposed against the premises. <br />1.3.6 REVENUE PROPOSAL (40 points) <br />The Vendor is to provide a percentage against gross sales, or minimum amount per <br />month, whichever is greater payable to the City on a monthly basis, for the following. <br />This will be per vending machine: <br />Example #1: If the percentage of gross sales payable is greater than the minimum <br />amount per month then the percentage of gross sales payable shall be submitted to <br />the City. <br />0 Example #2: If the minimum amount per month is greater than the percentage of <br />gross sales payable then the minimum amount per month shall be submitted to the <br />City. <br />Beverage & Snack Machine Revenue Proposal <br />To be <br />Submitted Via Bids nc <br />Percentage of gross sales payable to the City <br />Minimum amount per month payable to the City <br />RFP # RE-24-04 SECTION 1- INSTRUCTIONS Page 9 <br />
