Work Authorization No. 175 <br /> March 1,2016 <br /> Calvin,Giordano&Associates,Inc.,Proposal No.15-7567 <br /> • Prepare and report observed or identified deficiencies, <br /> unsatisfactory work,or work which does not conform with the <br /> Contract Documents. <br /> • Field review/verification of installed quantities; coordinate <br /> revisions(if required),and provide recommendation of <br /> approval of Contractor's monthly payment requests(total of <br /> 13). <br /> • Assist in the issuance of interpretations and clarifications of the <br /> Contract Documents requested by the Contractor including <br /> deviations from the approved design or specifications during <br /> the construction period/Contract Time. <br /> • Assist and evaluate the Contractor's requested plan deviations <br /> or material substitutions, and provide the City with <br /> recommendation(s). <br /> • Coordinate and assist in the preparation of any work change <br /> directives (field change directives), or change orders requested <br /> by the Contractor or by the City. <br /> • Review as-builts/Record Drawings on a monthly basis as a <br /> prerequisite of the Contractor's payment application. <br /> • Attend and assist in performing inspections for Substantial and <br /> Final Completion to determine, in general,if the project has <br /> been completed in substantial conformance with the Contract <br /> Documents. Prepare and distribute inspection punch list(s). <br /> Upon acceptance of the work,prepare a recommendation of <br /> project acceptance and submit to the City. <br /> • Assist in the preparation and processing of certifications of <br /> construction completion submitted in accordance with approved <br /> plans,permits and Contract Documents. <br /> 2. Construction Administration <br /> • Schedule,coordinate,and conduct one(1)pre-construction <br /> meeting at Municipality location;record,prepare and distribute <br /> meeting minutes. <br /> • Attend meetings with Contractor, City, stakeholders and <br /> appropriate regulatory agencies when requested and necessary <br /> SIB <br />
